Practice Pharmacist

Practice Pharmacists support the Nursing and Medical team in the delivery of safe and effective medication to our patient population. Practice Pharmacists are the point of contact with medication related queries on side effects, intolerances, supply issues and any questions that patients may have about medication. Clinics run during the working week, patients are booked into either face to face or telephone appointments to discuss medication for long term conditions, once they have had relevant investigations and observations carried out. Practice Pharmacists are Independent Prescribers and are able to prescribe new medication based on current diagnosis, amend doses and stop medication if clinically appropriate. They also process Discharge and Clinic Letters received from Secondary care and take the appropriate action. 

Alongside members of the PCN-wide Pharmacy Team, Practice Pharmacists liaise with the Business Manager and Partners to establish a route map for achieving QOF and IIF/Liaise targets as part of daily work, ensuring safety is maintained at all times.
